How to set up and run a survey

You do not sign in. Open the app. You land on Organisation.

1. Open the app

You see. The page title is Organisation. Across the top of the page are five steps: Organisation, Project & jurisdiction, Survey mode, Anonymity & schedule, Launch. The first step is current.

Press. Open Organisation in the left list if you are not already there. You do not press Sign in.

Worked. You can see the company list, Create company, and Country on the same page.

Stuck. A sign-in page means you are at the wrong address. Close that tab and open this app again so you land on Organisation.

2. Create the company (or pick one already there)

You see. On Organisation, a Companies list and a Create company field.

Press. Type the company name next to Create company, then press Create company. If the company is already in the list, press its name so it is highlighted. Do not create a second copy.

Worked. That name is highlighted in the list and shows in the header.

Stuck. An empty name will not create. If the wrong company is highlighted, press the name you want.

3. Countries, then states / regulators

You see. Under Country, four names you can press: Australia, New Zealand, United States, United Kingdom. There isn’t a country list box.

Press. Press every country this assessment covers. More than one is allowed. Press again to turn one off. Leave at least one on. Then press Continue until you are on Project & jurisdiction. Press the states or regulators that apply (AU states and Comcare if needed; NZ WorkSafe NZ; US states; UK HSE and the nations).

Worked. The countries you chose stay highlighted. Only those countries’ states and regulators are listed.

Stuck. Press Save draft, then open Project & jurisdiction again from the steps at the top.

4. Add sites

You see. A Sites page with a list of workplaces and an Add / edit site form.

Press. Open Sites in the left list. Press + Add site. Type the site name. Press the places that apply to that site. Press Save site.

Worked. The site is in the list when you open Sites again.

Stuck. If Save site does nothing, type a name first. Then open Sites again from the left list and check the list.

5. How people will take the survey

You see. On Organisation, the Survey mode step. Tick boxes for Open / shared link, Individual tokens, Shared workstation, and Paper survey.

Press. Open Organisation, press Survey mode in the steps at the top, and tick the ways people will take this survey. You can tick more than one. Press Save draft.

Worked. The ticks stay after Save draft.

Stuck. If the ticks are gone, press Survey mode again, tick them, and press Save draft.

6. How many answers before numbers show

You see. On Organisation, the Anonymity & schedule step. Three numbers: Answers needed for the whole organisation, Answers needed for a smaller group, and Answers needed for a department. Dates sit under that if you have them.

Press. Press Anonymity & schedule. Leave the first number at 20 unless you have a reason. The next numbers are for smaller groups (10). Fill the dates if you have them. Press Save draft.

Worked. The numbers and dates are still there after Save draft.

Stuck. Open Anonymity & schedule again from the steps at the top. If the numbers jumped, type them once more and press Save draft.

7. Launch the survey

You see. The Launch step, with Launch survey, QR for open link, and Poster.

Press. Press Launch in the steps at the top, then press Launch survey. The worker survey opens with this company name in the header. Workers use that page. QR for open link and Poster are on Launch. If you have no workers yet, press Test complete on the survey.

Worked. The survey page shows this company name in the header. After Test complete you should see a saved message.

Stuck. Open Survey in the left list. If Launch survey did not open it, that is the same worker page.

8. Results

You see. The Results dashboard for this company. It still opens before 20 answers. Some breakdowns stay hidden until there are enough answers β€” that is a privacy rule, not broken. PDF, Word, PowerPoint, spreadsheet, and Evidence ZIP sit in the header. Rahimi, Arnold, LaMontagne and Riley (2025) BMC Public Health is a reference table on this page, not a setup step you complete.

Press. Open Results in the left list.

Worked. The header name and the answer count are this company.

Stuck. If you see another company, go back to Organisation, press the company you want, then open Results again.

9. Optional

You see. Actions has a control register. Narrative (or Draft AI narrative on Results) has Generate, an editor, and Accept.

Press. Open Actions. Press + Add control, type the control, pick an owner and a due date, then press Save control. Delete sits on the same row. For a draft AI narrative, open Narrative or press Draft AI narrative on Results. Press Generate, edit the text, then press Accept.

Worked. The control is in the register with owner and due date. After Accept, the narrative is accepted. It uses grouped results only, not raw answers.

Stuck. If the control did not appear, press + Add control again and Save control. If Generate is quiet, wait a moment and press it once more. Skip this whole step if you are only checking the survey and Results.
